Another anti-TNF
Tocilizumab (Actemra) IV or subQ
Abatacept (Orencia) IV or subQ
Rituximab (Rituxan)
Add leflunomide (Arava)
Switch to oral JAK inhibitor
Stop anti-TNF and add hydroxychloroquine and sulfasalazine
