Case Report: Dizziness, Palpitations, and Shortness of Breath

History:

A man in his 40’s presents to the emergency department with palpitations, shortness of breath, and feeling a bit dizzy. He states everything started suddenly about 2 hours ago while preparing his lunch and he has never felt this way before. He denies any chest pain or pressure, syncope or near syncope, or other complaints.

Exam:

Vital signs were normal except for a pulse rate of 201. Physical exam was normal except for tachycardia and slight tremor.
