Undiagnosed diabetes mellitus in coronary artery bypass graft surgery
September 19th 2008We examined the prevalence and risks associated with undiagnosed diabetes mellitus among more than 7000 patients who had undergone coronary artery bypass graft surgery. At the perioperative and postoperative stages, patients with undiagnosed diabetes showed a significantly higher morbidity and mortality rate.

Stress echocardiography in diabetes mellitus
September 17th 2008We assessed how well dobutamine stress echocardiography predicted morbidity and mortality in 2349 patients with diabetes mellitus over a follow-up period of 13.2 years. Results showed that age, failure to achieve the target heart rate, and the percentage of ischemic segments were independent predictors of both mortality and cardiovascular morbidity. Using clinical and stress echocardiographic parameters, a simple model for risk stratification was developed.

Relationship between insulin use and development of hypertension
September 11th 2008This study evaluated whether exogenous insulin use to control blood glucose in patients with type 2 diabetes was associated with the development of hypertension by analyzing data obtained from a large national sample in Taiwan. Exogenous insulin use was shown to be a significant risk factor for hypertension development.
Statins are the most commonly used pharmacologic intervention in patients with increased cardiovascular risk. In addition to their beneficial effect on the atherogenic lipid profile, they have been shown to exert several pleiotrophic effects, including the reduction of low-grade inflammation. Thiazolidinediones (TZDs) are a new class of antidiabetic drugs that have been shown to improve insulin sensitivity and to reduce cardiovascular risk in patients with type 2 diabetes. Our study is the first to show a complementary effect of TZD and statin treatment on several cardiovascular risk factors in subjects without diabetes. These findings may have important implications for further discussion on cardiovascular risk reduction, especially for patients with metabolic syndrome.
