Frequently Asked Questions
What is Revita?
Revita is an investigational, one-time endoscopic duodenal mucosal resurfacing procedure developed by Fractyl Health. It holds FDA Breakthrough Device designation for weight maintenance in people with obesity who discontinue GLP-1 therapies.
How does Revita work?
According to Fractyl, Revita remodels the duodenal lining to restore nutrient sensing and signaling disrupted by chronic metabolic disease. Post hoc REMAIN-1 data suggest longer ablation lengths produce greater reductions in weight regain.
What did the REMAIN-1 pivotal cohort show at six months?
Revita met its six-month co-primary endpoint, with mean weight regain of 5.1% vs 8.2% for sham after tirzepatide discontinuation (P = .0023). Participants retained approximately 80% of their GLP-1-induced weight loss vs 67% with sham.
Revita efficacy in the REMAIN-1 pivotal cohort
The REMAIN-1 pivotal cohort is a randomized, double-blind, sham-controlled study conducted at >30 US sites. Investigators enrolled 308 adults with a BMI of 30-45 without type 2 diabetes (T2D) and with no prior GLP-1 therapy exposure. Each participant received tirzepatide until reaching at least 15% total body weight loss, then discontinued the drug and was randomized in a 2:1 ratio to a single Revita procedure or a sham endoscopic procedure.1,2
The study carries 2 co-primary endpoints in the modified intention-to-treat (mITT) population. The first is percent total body weight regain at 6 months with Revita vs sham. The second is the proportion of Revita participants maintaining ≥5% total body weight loss from pre-tirzepatide weight at 1 year, assessed against an FDA-mandated performance goal of >50%.1
At 6 months, least-squares mean weight regain was 5.1% with Revita vs 8.2% with sham (n = 176 vs 74; P = .0023). In a post hoc analysis, 32% of Revita participants were weight stable or continued to lose weight, defined as <2% regain, vs 18% of sham participants (nominal P = .020). The between-group difference widened at each assessment and nearly doubled from month 3 to month 6, according to the company.1
Dose-dependent response and safety of Revita duodenal mucosal resurfacing
Post hoc analyses by ablation length suggested a dose-dependent effect. Participants receiving 12 to 18 cm of duodenal ablation had an observed mean weight regain of 4.7%, a 43% reduction vs sham (n = 107 vs 74). Those receiving ≥20 cm regained 3.8%, a 53% reduction vs sham (n = 47; nominal P = .003).1
Fractyl noted these findings align with the REMAIN-1 midpoint cohort, in which more complete duodenal ablation drove a greater treatment effect. Prespecified key secondary endpoints include a complete ablation population (>14 cm), a higher run-in weight loss population (≥17.5%), and an optimized population meeting both criteria. Per the statistical analysis plan, these endpoints will undergo formal statistical inference only after assessment of the 12-month co-primary endpoint.1
As of the September 24, 2026, data cutoff, investigators observed no device- or procedure-related serious adverse events, no deaths, and no unanticipated adverse device effects. Related adverse events were predominantly mild and peri-procedural, with 94% occurring within 7 days of the procedure and most within 2 days. Fractyl characterized the tolerability profile as consistent with upper gastrointestinal endoscopy and supportive of potential broad outpatient use.1
“The success of the REMAIN-1 pivotal study in its first co-primary endpoint establishes the thesis that targeting the duodenum may enable a durable metabolic reset for millions of patients who seek to treat the root cause of their obesity rather than rely on medicines to manage the disease,” Harith Rajagopalan, MD, PhD, cofounder and chief executive officer of Fractyl, said in a statement.1
Fractyl plans to submit a De Novo marketing application to the FDA in late Q4 2026, with topline 12-month pivotal data, including the second co-primary endpoint, expected in Q1 2027. A Category III CPT code (1122T) for the procedure takes effect January 1, 2027, and the company is targeting a potential US launch in early 2028. Revita remains investigational in the United States and is CE marked in the European Union and United Kingdom.1
