TELIGAN: Telitacicept Preserves eGFR Through 2 Years in IgA Nephropathy
Final TELIGAN data show telitacicept cut confirmed 30% eGFR decline by 89% and reduced proteinuria by 60.7% vs placebo over 2 years in IgA nephropathy.
Telitacicept, a dual B-lymphocyte stimulator (BAFF) and A proliferation-inducing ligand (APRIL) inhibitor, slowed kidney function decline over 2 years in adults with
The annualized eGFR slope was −0.6 mL/min/1.73 m² per year with telitacicept vs −5.7 mL/min/1.73 m² per year with placebo. The data were presented during the Featured High-Impact Clinical Trials plenary at
“At 104 weeks, TELIGAN demonstrated a substantial reduction in proteinuria and important evidence of durable kidney protection,” Jean-Paul Kress, MD, Chairman and Chief Executive Officer of Vor Bio, said in a press release.2 “An annualized eGFR slope approaching zero, lower rates of kidney function deterioration, and sustained proteinuria reductions following the transition to every-other-week dosing together reinforce the clinical potential of telitacicept. These findings add to the growing body of evidence supporting dual BAFF/APRIL inhibition as we continue to advance our global Phase 3 programs in myasthenia gravis and Sjögren’s disease.”
The 104-week results build on the 39-week stage A analysis, in which telitacicept reduced 24-hour urine protein-to-creatinine ratio (UPCR) by 58.9% vs 8.8% with placebo (P < .0001).³ Those results, presented at ASN Kidney Week 2025, supported conditional approval in China in June 2026 for reducing proteinuria in adults with primary IgA nephropathy at risk of progression.²,⁴
How Did Telitacicept Affect eGFR Slope in the TELIGAN Trial?
TELIGAN randomized 318 adults with biopsy-confirmed IgA nephropathy on stable supportive therapy in a 1:1 ratio to subcutaneous telitacicept 240 mg or placebo.¹,² Treatment was given weekly through week 39, followed by a maintenance regimen of every 2 weeks through week 104. RemeGen conducted the trial in China, and the annualized eGFR slope over 104 weeks served as the stage B primary end point.²
The between-group difference in eGFR slope was 5.1 mL/min/1.73 m² per year (95% CI, 3.5–6.7; P < .001).¹ Confirmed eGFR decline of 30% or more occurred in 2.5% of telitacicept-treated patients vs 16.4% of placebo recipients (hazard ratio [HR], 0.11; 95% CI, 0.04–0.30).
Confirmed eGFR decline of 40% or more occurred in 2.5% versus 9.4% (HR, 0.19; 95% CI, 0.06–0.56).¹ No patients receiving telitacicept experienced a composite kidney failure event, compared with 3.1% of placebo recipients. According to Vor Bio, the placebo group had 5 such events, 2 of which required dialysis.²
Were Proteinuria and Safety Outcomes Sustained With Telitacicept?
At week 104, the placebo-adjusted UPCR geometric mean ratio was 0.39, a 60.7% reduction (95% CI, 52.6%–67.4%).¹ According to Vor Bio, 44.0% of telitacicept-treated patients achieved UPCR below 0.5 g/g vs 8.2% with placebo, and reductions held after the switch to every-other-week dosing.² Serum galactose-deficient IgA1 decreased by 47.8% with telitacicept and increased by 19.1% with placebo.¹
Adverse events occurred in 95.0% of telitacicept-treated patients vs 89.3% with placebo.¹ Serious adverse events (8.2% vs 18.2%) and serious infections (1.9% vs 4.4%) were less frequent with telitacicept, and overall infection rates were identical (67.3% in each group). Injection-site reactions occurred in 56.0% vs 18.9%, mostly mild to moderate, according to Vor Bio.² No deaths occurred.¹
Telitacicept is approved in China for IgA nephropathy, systemic lupus erythematosus, rheumatoid arthritis, generalized myasthenia gravis, and Sjögren’s disease. Vor Bio is conducting global phase 3 trials in generalized myasthenia gravis and Sjögren’s disease and has not announced US filing plans in IgA nephropathy.²
